- 6 year old Female
- Active recall patient at our office
- 6/6 Red flags for Paediatric SDB – mouth breathing, enlarged tonsils, tongue tie, narrow palate, evidence of wear and retrognathic mandible
- noted snoring, grinding, severe crowding in primary dentition
The signs were all there, patient was screened and the recommendation for Early Interventional Ortho by skeletal expansion was discussed. The goal is bettering the anatomy in order to promote nasal breathing, make space for the tongue and allow for normal growth to resume. There is no orthodontic alignment at this time.
11/11/2024 – iTero Scan
- Severe Crowding
- no Leeway Spae
- no room for #31 or #41 (90 degree rotation)
- SDB signs and symptoms

3/20/2026 – iTero Scan
- Crowding continues
- Note #42 and its SEVERE lingual position – NO ROOM
- SDB signs and symptoms continue
- mandible is more narrow

Conclusion
Depending on the goals for this patient, early interventional orthodontics is an excellent treatment option. In fact, starting earlier would have been ideal. The risks of no treatment include needing to remove healthy adult teeth for ideal alignment, possible jaw surgery, unideal occlusion and lifelong airway and breathing issues.
